You'd be surprised how many mods work fine with TTW, but some don't, and they can break the absolute fuck out of your game. For continuity's sake, you should probably start in the vault. In other words, you need to be born into vault 101 the first time you enter the capital wasteland, and you need to get shot in the head the first time you enter the mojave. The game needs to go through the opening sequence to properly set up for each wasteland.
This process is not difficult or complicated, but it will take a while. You will need to download an executable from the TTW website, patch your own legitimate copy of fallout 3 into a mod for new vegas, and then install it.
